Special requests in preparation for your clinic visit:
Brown bag your medicine bottles. Bring all your medications to the clinic visit.
Bring your insurance information. We need to see an original insurance card.
Bring your patience. We try and see everyone in a timely manner. Appointments and minor emergencies will have priority but delays are inevitable. If your visit is being overly delayed, please feel free to flag down a staff member about your concern and that you did have an appointment.
Bring your payment. All co-pays and cash payments are expected at time of service. We do take economic hardship into consideration on a case-by-case basis but it is hoped you do not under-value the service we provide as we need to pay our bills too!
To expedite your visit, you can call us and ask for us to fax your intake questionnaire forms for you to fill out before arrival to the clinic. This could save 1/2 hour off your visit time.
